In a medium saucepan, combine the sweetened condensed milk, cocoa powder, butter, and vanilla extract (if using).

Cook over medium heat, stirring constantly with a wooden spoon, until the mixture thickens and starts pulling away from the sides of the pan, about 10-15 minutes.
